September 16, 1945 - January 28, 2017
Left us peacefully while in the care of the 10th floor SAMU/Palliative Care Unit at LHSC University Campus. Jay's memory will live on in his wife Sandi of 47 years. His smile and wisdom will live on in his children Natalie (Dave) and Nicole (Ricky); grandchildren Keilyn, Clayton, and Karlee; adopted/honourary family Kelly, Lavalle, Sienna and Cohen. Survived by his sister Robin. Jay's humour and many "points to ponder" will be forever remembered by family, friends and colleagues.
A Time To Remember will be held for Jay on Friday, February 3, 2017 between the hours of 3:00pm and 6:00pm for those wishing to pay their respects at FOREST LAWN FUNERAL HOME, 1997 Dundas Street (at Wavell). The family would like to give special thanks to Dr. Ballut and his team of angels for their care and compassion. In lieu of flowers, donations in Jay’s memory can be made to the CNIB, Canadian Diabetes Association, or the Kidney Foundation of Canada.
